If you have things to explain ,you can always add an attachment sheet since sometimes the forms don't have enough room. I practice in California so can't help you with New York law or procedure.

A few thoughts:
(a) Childcare costs. Will your children be in childcare while you work? This is usually listed as an expense for the custodial parent, so he may have to pay that if you are shouldering most of the childcare costs
(b) Health insurance - yes, put in "future" expense, although my guess is you need to list the kids only when factoring in health care costs. I don't believe he would be responsible for paying your costs, just the kids.
