Depending on what kind of retirement fund it is, you should be entitled to immediately "roll it over" into another personal retirement account, or "cash it out". Please remember that if you choose to cash it out, you will have to pay ordinary income taxes on it and a large fine for cashing it in before retirement. Talk to someone who has financial experience about it as the fine can be as much as 25 percent.....
You may also consider directly contacting the company representing your fund to ask to have the release of funds expidited...
File a claim in small claims court with whoever is responsible for not processing or delaying your $$. Pay the extra $50 or so to have the summons delivered by a sherrif in person. Then see how fast things start to move.
Of course you will probably alienate most everyone involved, but you will have proven that you are as immature as the best of them and that you're not afraid to take care of #1. But, again, my approach is probably not the best one. It is, however, probably the most fun.
I would write a letter to accompany the form indicating that you were not provided the form to file upon your departure, nor were you advised that a form would be necessary to request your monies. As a result you expected to receive the monies in a timely manner and because you didn't you now need to request that they be disbursed immediately because you are encountering financial hardship.
