This is a great question. There are certainly some gray areas.
Since many of these doctors are very specialized, they probably don't do too much outside of their area of expertise. This is not to say they couldn't provide you with some excellent guidance. It probably just depends on the practitioner.
If your situation is such that you're not needing surgery, a neuro or orthopedic surgeon won't be able to do too much for you.
If you have a primary care Dr who is good, perhaps they can help direct you.
I've found that exercises that strengthen "core" muscles help my back pain greatly.
My wife was experiencing some bad lower back pain which was caused by spinal conditions. She went through pain management which was no help. She just had surgery and has not yet come home from the hospital. I see what she's going through. It's a tough surgery. Once she recovers, I hope she will be more vigilant about exercise and losing some weight.
This is all fairly new to me and I have my primary doc and the pain doc.. I hope you can get some answers as it sounds very confusing..

I start therapy next week I believe, as my 2nd shot is thursday, Doc wanted me to start PT after that..
