If your only option is surgery then that's what you should do. Synvisc though is suppose to work for an extended period of time...it's worth a shot and if it doesn't work then go for surgery.
You should be taking some kind of meds and I highly recommend Serracor-NK, which is good for both RA and Osteoarthritis. You can look at both of the case studies. It's worked really well for me but I don't have the kind of damage you do.
