If this anxiety has come back out of the blue, there is a reason for it. Again, I think anybody who goes through it should get a medical clearance before chalking it up to anxiety, but those cases are the exception and not the rule. While you are waiting for that appointment, I wouldn't spend the time focusing on the physical possibility. I would focus on the mental because that is a FAR greater likelihood. Chances are your brain is sending you signals that you have some emotional issues you need to deal with. Whether it is cumulative stress or one specific event, only you can know, but I would give it some thought and see if anything comes to mind.
And yes, hormones definitely make it worse. Mine is always worse around that time as well.
