My sister recommended me to a doctor that was very kind and patient and FIVE root canals, three cavities and four crowns later, I am out of pain! I did it with taking klonopin before I went to calm me down, then gas when I got there. I was also referred to a dentist to knock me out, but the costs was almost too prohibitive. I really felt if I could trust a doctor to help me feel in control, I could do it with pre-meds and nitrous.
Really, half the battle is making that bond with the doctor. When you feel he is sensitive to your fears, you can go much further than you ever thought you could.
Remember, our minds ALWAYS make the situation worse than it ever is and you are going to have the greatest payoff ever. You'll be out of pain. So, you need to say things like this to yourself, "In two hours, all this pain I've been going through will be over. I can certainly endure anything for two hours that stops this misery.
Also, be aware it's VERY common for dentists to pre-medicate before appts. That is, to give you a script that is for medicine that calms you down. They will usually give valium. It works wonders.

Wishing you the very best. I always have anxiety at the dentist. I have periodontal disease and have to get those DEEP cleanings that I dread. Today I went and practiced breathing and visualization and telling myself that I could get through it. I did so good! I even had cavaties filled. My blood pressure was only 133/79 which is great for the dentist. My BP normally goes sky high when I go there.
