I agree with the idea for list making. I find that making a list of things I want/need to accomplish for the day gets me on task better than anything else.
You could try starting a 'bullet journal'. They seem to be all the rage recently. It's just another (actually really neat) way to keep track of things you need to do, things you have done, and take notes about things you want to remember. I think their official website is bulletjournal.com
However, I also have a problem with lists that are too long or have too many big stressful tasks on them. Then I just panic and get so overwhelmed I can't do much of anything. I have a big problem with that. I've almost developed a list-phobia.
