I think it would be best to start back slowly lovie, as you said you dont want to end up back at square one again.
Do you have coping startagies?
do you have a good support system?
There maybe free groups in your area for anxiety and deppression, ask your doctor, it could provide you with great coping skills and you wouldn,t feel so alone in all of this.
Im in same position as yourself, im dreading going back to work because of my anxiety, alot of my fears are work based, so im working with my physcologist on this.

Instead of dreading the end of your leave and going back to work you might want to try going the positive thought route. Look at the first day back to work as a challenge. Tell yourself you can handle it and you'll be back to your old self in no time. It would be nice to get a new job but you want to go back to the old job just to prove you can do it!
If you say stuff like that to yourself it truly does help. It takes time though so don't give up after ten minutes.
Another reason to go this route is I find anxiety is almost like an addiction. The more you give into it the more intense it becomes and the more it wants.
