My counsellor recommended a book for me called Hope and Help for your Nerves (I'm about halfway through and it is the best book I've ever read).
I don't have the same fear you have about choking and swallowing, but there is a section in my book that discusses fear of swallowing (nothing on choking though, sorry).
The book states that you don't need to worry about trying to swallow; simply keep chewing. The swallowing will look after itself as it is carried backwards. It will eventually find its way backwards in spite of your nervous resistance. If you keep chewing, the food will all eventually disappear.

I think many of us here feel our throats get tight occasionally. That's a big part of anxiety for a lot of people... But you need to eat. If you don't you will continue to feel terrible.
