Attention deficit/hyperactivity disorder (ADHD) and attention deficit disorder (ADD) are more common than you might think. See if you can get people to help - I got my kids to sort out my store room a coupls of years ago - gave them carte blanche to throw away what they wanted.
Are you on meds? makes a big difference to me.
Try and focus on one thing at a time - e.g. if you're making supper focus on that.
prioritise - make a list of the things you think you should do, prioritise them and see which of them you can farm out.

I find it very useful (and productive!) to put things running through my head on a list. When I handle the item, I check it off as completed. I find that if I write down the numerous things that run through my head, I don't panic quite so much because I can see the things I need to handle right in front of me.
I'm queen overwhelmed.
I make a point to stop myself when I'm feeing this way.
Find a quiet place, sleep.
Get up the next day, make a list of tasks I'm going to accomplish, hang them up, cross them off one by one.
ONE STEP at a time, and I know it's cliche, but it's true.
I get so up in my head, and ahead of myself that sometimes I just have to step back, let it go.
Luckily in therapy[y, I'm allowed to verbalize every thought, related or unrelated that comes into my head. I'm sure she thinks I'm crazy.
