the mind changes, my mind changes but no matter
what the issue I have to remind myself to take care of myself.
I don't know what the purpose of these mental underminings is
nevertheless I have to keep to the essentials and keep managing.

I can think of a few essentials you display a lot of people could learn from mark. You are a kind and generous sort of guy with a wonderful streak of genuineness that makes it a pleasure to call you friend. Give yourself a pat on the back mate. You are a good guy.
I like the phrase, "mental underminings." Consider that one stolen on the Twainian justification: If it wasn't for plagiarism, we would have no culture.
The psycho theory which resonates most in my own skin is that those "mental underminings" is that a childish drive to understand forged a neural pathway which was never resolved and is still spinning to find its connection. A case of a solid instinct turning to destructive ends. Kinda like a mean scratch in a DVD. I go with the brain plasticity theories that, on a bio-disc, it is possible to partition off the damaged section and route new neural pathways. Learning a new language is their favorite tool for forging new neural pathways, and even if it didn't work as well as I *think* it did, at least now I can obsess in three languages. Working on the fourth. . .
At least I *think* that's what the folks at the Stockholm Brain Institute were trying to say. . . Whether I'm quoting them well, or knot, that's my theory and I'm sticking to it.
Keeping to to the essentials and managing gets my vote as the foundation of healthy living, whatever your preferred theory for the sticks and tics of life.
