got my appointment at the neuroscience clinic, september 6th (lucky number is 6!!!) at 10:00. This is a teaching hospital, so I am prepared (and looking forwrd to) being used as a guinea pig! just means they are the most likely to dx me!!!!!!! woo hoo feel good today

glad you got your appiontment, take a piece of paper write down the following information. Then put it on your computer and save.
Name, address , phone no#
birthday
ss number
hospital
All doctor that treat you and their number.
blood type
insurance information- Id----, Group#-----, Plan--- Eff date---
Medicine List--all medicine, doses, how you take
Reaction to any medicine you have
Surgery\'s---what kind, date
History\'s -----all things that has to do with your health.
Make separte list of your medicine-- for your visit
you can do all of this on your computer and print off for your visit
you can update this at each visit .
so the next time all your have to do is print it off for your doctor.
This way you don\'t forget anything and it it all right there.
