As far as having channels of communication, see if there is an LGBT group somewhere near by that accepts teens. Often schools will have them and colleges as well as community based groups often exist. It would be a safe place for you to go and get questions answered more specifically to Canada. I have known a number of trans individuals who transitioned in Canada and I know that the rules somewhat change from province to province, but for the most part Canada as a whole seems pretty cool with it overall.
From the standpoint of coming out and passing, coming out will be really when you want to do so and how you want to do so. Passing is an odd duck in that you can be the most manly man out there and actually still have a decent chance of passing. Passing is more about knowing absolutely you are a woman and not a man. From there you are free to move about the cabin so to speak. If you go out to a mall and sit on a bench for hours and watch people you will see women who are strong, women who are weak, long hair, short hair, makeup no makeup, very feminine outfits, masculine outfits, large women small women, butch women and the girliest of girly women. The same goes for men, the spectrum is very wide and it makes it very easy to fit in. You just have to know without a doubt what you are. if you aren't thinking am I girly enough or am I passing, no one else is going to think that either. I myself favor my father in looks as does my sister. Neither of us are ever called sir in public (I am occasionally on the phone because I don't always pitch my voice and leave it very deep, even then I get called maam more than sir). You need to get comfortable with who and what you are the rest falls into place from there. You list you already have all these feminine traits, so is it really that hard to just say you are a woman and not a man? Sure you have the wrong parts, but those are parts. Whats inside is what makes you a woman, not your body.
Last part implants and such. This one is a bit of a challenge for me to answer because I don't know how things work in Canada medically with elective surgery. IF you were to get on HRT right now and either stave off puberty or get full HRT for the female side of things, your body at your age will largely develop as it would if you were born a girl at birth. That said it is dangerous so do not attempt it without a doctor (again this means therapy). At that point waiting it out you wont need implants. If you need implants its an outpatient procedure, you walk in they do the thing and you walk out wearing a special bra for a while. You and the surgeon will decide which method and type of implant are best for your goal (under the muscle or over the muscle, saline vs silicone so on) Each type and method has their own positives and negatives. Now, I was 32 when I started my HRT, If you are going to go the route of HRT DO NOT GET IMPLATS!!! your boobs are going to grow on their own. In my case I went from nothing to a small c cup, most the women in my family have D's. If you got implants now to C and had growth like I did, you will be paying for another surgery to have them removed and the skin cleaned up. DONT DO IT. wait for 2 years after HRT before considering it. If all you want is something to give you the illusion, there are things called breastforms that you can buy to give the illusion of boobs. They are expensive for good ones but there are ways to make them relatively cheaply with balloons as well for crossdressing in private.
Facial surgery, there is a ton of things you can have done, but again you likely wont need it. When push comes to shove, you will pass just by being you when you can get to believing in you. If you still want to seek getting things on your face altered, you will need to discuss it with a surgeon and deal with any costs associated with it. Both breastgrowth on HRT and facial structure is based on genetics and it is very specific to each person. Things you can have done on your face are immense. You can have every single thing on your face worked on, touched up, and altered to a certain degree, so its really going to be based on how vane you are. Just remember you are altering a forever changing canvas and as a result, you will always be chasing a beauty standard you may never meet because you age every single day. My advice is come to terms with your looks. learn to work with it and go. I don't recommend surgery at all. Implants after HRT has done its thing is one thing, altering your entire face, not unless there is a real medical reason. In the states there is a significant financial investment required for surgeries, that may not be the case there so you will have to look into it from a Canadian stand point. In any case all surgeries would require your parents consent so that wont happen till you come out or are 18 anyway.
Hope this helps, I gave you all the information I can think to give. get to a therapist and get that step done. you will need letters and all for implants and HRT and all anyway so do that now and work on you inside, worry about the outside later.
