Whatever T is like when I awaken is what I have to live with for the day. Even if I don't sleep at all, the dawn of a new day is the dawn of a new T day.
Currently I will have a bad day(such as today) with loud practically debilitating hissing, followed tomorrow with lighter hissing, the next day with very light soft hissing, and then the routine starts over again.
I have learned to manipulate this T routine. If I want a good day instead of a bad, I can do as follows. Do something to spike my T on a good day(sodium, sleep deprived, get stressed(that part is easy) and that would change the pattern causing the good day to be bad and visa-versa. Every ENT would be happy with me, as I've learned to "just live with it". Dang it anyways.
When you think about it, don't arthritic patients suffer in a similar fashion. They "just live with it".
There are things worst then death, but this ain't one. Wish all-good health.

If I have any relief and get better - I will post a diary here - so others who are suffering like hell can wonder if the money and time investment is going to be worth it.
Five people have posted here as having success after six months - so it would be summer before I could honestly tell people if it is helping.
My t is very strange. Some days it will be very quiet, other days it starts off quiet and then by bed time it is louder and yet other days it starts off loud and will stay loud until the next day. It kind of has a mind of its own, if that's possible. I am learning to adapt to whatever t day I have and on the louder days I will evaluate the day before to see if I did anything different or ate anything out of the oridnary that might have triggered the t to be louder.
I have read about Xanax, but have decided not to pursue this drug yet as I don't experience anxiety because of the t and the ringing isn't that debilitating to me. I pretty much just live with it. I've had occasional drastic spikes in the level of noise and I just pray and it goes back to "normal."

I'm really sorry as its a very tough time for you, that day in and day out feels like a raging forest fire. Its and interesting analogy as what is needed is the method to quench out this fire.
As you are active on this site and evaluate much research, I'm sure that have considered just about everything. Therefore please excuse most of the following thoughts.
Currently you're navigating within the T circle. The T you have is causing anxiety and hence that causes more T. I've been there myself and I really feel for you.
So we need to get you off this repetitive nightmare and make T not important anymore in your life(not important is the key). It appears that it would be great to find a way to just relax a little as well be able to get some sleep. I would speak to your MD to see if they can hook you with a med that is not addicting, safe and that has no other side effects. Hopefully after a short time, you'll begin to get the necessary rest, the anxiety will be reduced, and the ringing/hissing will be lot more tolerable.
Ok the ringing may not vanish, but it becomes only annoying like sayyyyy a hemorrhoid. So how do deal witt something like that.
Keep busy, which for me consist of work, taking care of domestic responsibilites, getting and giving support within DS, and most urgent (with a star) is to begin to do things that you enjoy. Maybe its window shopping or shopping, travel, any hobby etc. You've been through a lot over the years and its time to say: I want to do something for me. Doesn't have to cost much as the basic fun things usually don't. This may have a huge impact in getting off the T circle, making it not important and regaining a quality of life.

Stress affects me big time, and, my weekends or days I don't work are usually better, but not always.
I have noticed another trend: When I take Sonata late in my sleep--say around 5 AM my T is usually better the rest of the day. Sonata (Zoleplon) has a very short half-life and therapetic affect on sleep of only about an hour, so, it's used mostly for people to fall asleep. However, there's something interesting between this medication and T, IMO.
