I do understand what you are going though I myself am a huge procrastinator. I take time to plan and most of the time I don't bother to look at my planning; so I have also challenged myself to get into the habit of checking my schedule every week. Some weeks I am on point and other weeks not so much. Nevertheless, I keep at it, You should too!

Also, try using some tools to help you organized and on track. For example, project management tools, time tracking software etc. I personally use WebWork Time Tracker, which helps me track time on projects, assign me task, see how I perform them, how much time I spend on them, how productive I am.
