I've been thinking about this question a lot lately. Over the past few years as my pain has progressed, so has my list of meds. I have to keep a daily journal so that I can write down exacty when I take each thing in order to keep from accidently taking something twice...or missing a dose for fear that I may have already taken it.
I keep everything in my private space of the house...I live alone so I guess the whole house is my space, but sometimes it feels like a hotel lobby. My whole family lives about one good spit away!
I have a terrible time getting any decent sleep. I was up every hour and decided to give up at 2:45 this morning. Something in my med cocktail is causing me to have dizzy spells, sudden, overwhelming sleepiness, nausea...all sorts of bizarre things that I can't identify the source of.
When I saw my doc Monday, I mentioned my concern about Serotonin Syndrome. She said that only 2 of my meds could cause that, but didn't seem the least bit concerned! Of course, she's not the one who has to look at a long line of prescription bottles on her desk and try to plan the doses to enable her to work a little.
Yesterday, I decided to find a site that could look at everything I take and report any that might interact. It was more than a little disturbing to find 11 warnings with one being hilighted as 'very serious...find alternative'. Four of them had serotonin warnings when combined!
I never know how I'm going to feel at any moment of my day. My pain level is pretty high right now, too. This even though my first dose of pain meds has had an hour in my system. I often wonder if I feel worse because of this ridiculous cocktail or my condition is simply getting worse and the meds aren't enough.
I would really like to start over and find out exactly how my body would feel if left to fight pain without help. I know I can't stop everything or I'd really be in trouble, but I also can't imagine living like this for the rest of my life, either. I have to work so I don't have the luxury of taking a chance on more pain...it's a discouraging mess andI don't know what to do.

My 6mo twin DD's are EBF and just started rice cereal on the first. Everything has been going fine except this morning one of them had a diaper rash (lots of redness around her anus). I have found out you can have blood work to test your serotonin level when you are on meds that can cause the syndrome.
I was taking a large amount of B12 and usually in the afternoon. Big problems with sleep. I learned it should be taken in the AM. Also some supplements actually don't work with our meds.

If I feel that i'm not improving at all within a few mths then I have a big discussion with my dr about it. It is time for a change. Yes we can get worse, but if the meds aren't helping why put our bodies through the stress of them? At one point I looked at my meds and I couldn't believe all that I was on and I wasn't feeling better at all, so I told my dr I wanted off of several of them.
I hope you get the answers you need soon.

It is a tough transition. Took me about 2 months. But I feel so much better now. Actually can finish a sentence and plan a shopping trip. Pain is better too which seem counter to what we expect when reducing the amount of pain meds.
You do need a doc who will listen or has a Nurse practioner who takes the time to listen.
