As far as the self-harming bit goes, I understand how difficult giving it up can be. I know it's challenging, but there's a lot of coping mechanisms that can really help. I know they sound so typical, and you've probably heard them before, but holding ice, snapping elastic bands, and clapping hard can sometimes help with the sensation.
I know long distance friendships can be hard to maintain, but if they're the only person you feel like talking to, it's obviously worth the effort. Try to text/snapchat/facetime daily, even if you don't have much to talk about. If you can, even try to send them letters - the sentiment is nice.
As for the breakdowns/lack of interest in everything, maybe you should talk to your parents. I know you said they're not exactly supportive, but if your medication isn't working, maybe they can try to help.
