Believe it or not Walmart has glass etching at their photo labs. An ornamenet is one of the options.

I know Lennox have ornaments that you can personalize. My sister ordered one for my son that will be personalized with his name and birthday. I liked the ornaments from Lennox because I was looking for something that was not to Christmasy since my son was supposed to be with us for Christmas but sadly won't be. For Christmas last year I got Madeline a beautiful ornament from them. It is a silver pair of baby booties with her name and the date engraved on the bottom.

After my girls passed away, the doctors and nurses came in with pamphlets and keepsakes from different foundations. And since it was December when they passed away, I took the keepsakes and gave each baby(I had triplets) their own little ornament by tieing them together. They now have their own little section on my tree, right in front.
