i wouldnt say sweetheart was a flirting word as i know loads of people who say it, i think if he said 'gorgeous or babe' he would definately be flirting. but it isnt really professional :/
BUT if you dont feel comfortable with the doctor then change!!
I prefer professionals to speak to me as a person and not their title - but Calling you sweetheart is in my opinion crossing a line, Although I had a support worker who ALWAYS referred to me as love, but also referred to others in the same manner - if he is a good doctor I would take someone with you to you next appointment like you mum or sister - see if they think he is being out of order.
But ultimatly if he is making you uncomfortable you should change doctors, (just ask to see a female if you are worried about making him feel bad, being female you entitled to this without question).
