I'm wondering if your back could be contributing to any of this.
The spine is complicated & more often the not sends pain to other areas of the body.
Another thing to keep in mind if one area of the spine such as the lumbar is problematic then you should keep an eye on the others, cervical & thoracic.
On top of the RA my cervical causes pain in my shoulders, fingers etc... Lumbar at times my toe
