I'm sick. Unfortunately i dont know the specifics of it beyond I'm sick. Even after a 45 minute visit with the NP who is taking my PCPs patients while he's out until Nov. (He's gonna get an ear full!)
I got a sore throat last week. Wednesday. it got progressively worse over the weekend and Sunday night i developed a cough. So pull out my "Im sick stuff." Nebulizer, albuterol, cough pearls, delsyum and tylenol oh and a bucket for the middle of the night.
Monday i went to school. Felt miserable but i had an exam i couldnt miss.
So Tuesday we called the doctor and my primary is out until November...totally wish i knew so i could tell my body to wait to get sick lol.
I see the NP. This afternoon. She had the worse bed side manner ever. I get that she doesn't have time to build a rapport but when you have a chart that says 3/3 doesn't it help to at least look at it slightly before had to know a miniscule of their history. I get time is short but still if you don't have time to read then ask.
So she asks about a bajillion questions, questioning every medication im on and then gave me this horrible look when she got to the MScotin and the folic acid, and the vit d and the methotrexate. The nurse who did my vitals was wonderful and i know her she knows me and just simply marks i still have the MScontin even though i dont use it often.
Then she asks why i'm there for which i said "I had a sore throat last wednesday" she interrupts "You mean yesterday?" "No last week. then a cough developed sunday. I'm not congested in my nose. I'm short of breath. I am asthmatic."
She asks why im on the last three drugs mentioned above and i say i have "RA." and leave it at that i have more but she was being rude.
I get that each Doc and NP have their own way of doing things but it irks me when someone gives me that look that im perfectly fine and nothing is wrong.
So she starts the physical assessment, which was not exactly fun but not horribly either. Looks in my ears, nose and mouth. Notes my throat is red but not got any white patches. tonsils are inflamed and my lymph nodes are tender to the touch.
Oh and she asked if i was running a fever, which i should note i dont run fevers often. But in the last week i have been. I dont know the actual temp because my thermometer died but i know what i feel like when im fevered.
So all in all...no idea why im coughing but its viral. and my sore throat that started before the cough i might add is caused by my coughing...damn apparently my throat has perfected the art of time travel...now.
She gives me a inhaled steroid. Advair. I am against inhaled steroids, the powdered form, specifically Advair.. I told her this and her response was "I'm not giving you oral steroids" Fine whatever, they did a rapid strep test was negative but are culturing it just to make sure.
My issues with advair stem back oh hell freshman year of high school. So..08ish i think i was on it. I was a new asthmatic and getting things under control. So around end of march i got a cold, not a big deal. Except it quickly turned from a cold to the worst lung infection i've ever had. I spent 8 weeks with a lung infection from hell. 8 weeks. 9 different antibiotics. Weekly trips to the Drs office for a check up, he was trying to avoid the hospital as he felt i would get worse there and catch something new. I was running a constant fever of anywhere between 99.9 and 103.5 i would go to school if it was below 100.0. So it wasnt often but i went. I didnt eat, i didnt sleep. I could barely breath. We get to week 8 and my parents and doctor are getting concerend and they are talking about taking me to the childrens hospital but my doctor was still seriously worried that i would get a bigger infection going there and he had one final option for an antibiotic and if that didnt work, he would help get me into childrens hospital. There is only so long the body can work at the o2 stats i was at before organs could shut down.
At this point, i should say I love my pcp.
So i get this irradicated..i spent all summer healing but was thankful that i got better and have minimal scarring from double pneumonia.
My doctor spent weeks researching it before we conclude that the cause of the serious lung infection was the Advair discus. And we begin the process of moving away from any type of inhaled steroid as he felt it was only a matter of time before each one causes a repeat.
So the fact that i got sent home with a sample discus to use for the next two weeks is upsetting and rather scary. I have asthma. it only exacerbates like it is now when im sick and need a 10 day round of antibiotics. And i hate antibiotics. My pcp hates that i need them. My body needs them twice a year. Its how its been for the last 15 years. We try the non antibiotic route until last year my pcp was like why make you suffer if we do the round of antibiotics you get better faster and in the scheme of things. While i know antibiotic use is detrimental. I cant go 3-5 weeks unable to breath as well as my baseline.
But she didn't want to listen today. it was like she knew better than my pcp and i, who have been working together for the last 17 years we have a good rapport. We know what works and what doesnt.
So i am doing breathing treatments every 4 hours and at times they arent enough. I feel wet in my lungs.
Oh. She didnt hear weezing either, she spent a good 5 minutes trying to find some. She'd have a problem if i was wheezing. And she never did check my O2 stats. i dont even know where im running.
If im not better by next tuesday i am going to call my rheumatologist maybe she can offer in sites.

ask the pulmonologist what he thinks of mtx and you. i had to part company eventually, cause of my lung infections.

Its a fear i have as well which is why when im sick i feel the need to advocate harshly for it.
I also understand that in a not so complex person an inhaled steroid is the way to go and they work. But they scare me, which is why i didn't use it. Could it have worked? Probably could have but too me the risk of getting a worse infection was too high. I went to a local walk in clinic and got a cough syrup via a RX that works extremely well when the other cough pill i has doesnt work. Im feeling a loads better in terms of my lungs.
Now im back to my throat hurting and my tonsils inflamed same with the lymph nodes in my neck.
As for the nurse practitioner i saw, i understand that im a complex case. I stump my pcp and rheumy at times. I have some overlapping which is hard to figure out what is the actual cause of something. And I get that she has an opinion, she can have her opinion but when it blatantly goes away what my pcp says as well as my rheumy is when i turn off and its not intentional. I have a lot of information thrown at me sometimes.
I've worked hard to come to the three primary diagnosis i have, they all overlap to a degree...isn't that nice when they do that? ^.^
I have Asthma, Rheumatoid Arthritis, and Sarcoidosis. On top of that i have Multiple drug allergies, to not just antibiotics but also other drugs.
And i still have some unexplainable issues and some that i have yet to get to addressing. Its defeating at times and adding an acute illness at times just makes it worse.
I've only ever had to lodge a complaint against a doctor once because he didn't take the time to read my chart. He tried to give me medications im allergic too.
I want one month where I don't visit a doctors office. Get a call from a pharmacy to schedule drug deliveries, go get blood work. Go get scans of some kind. But alas i cant. So whats the point in wishing.
