...and report him to his superiors. If he is his own boss, report him to the state. This won't make much difference, really, but it may one day be important that you protested your treatment.
I would go back to your PCP, and have them treat you, and do lots of research so that you can help your PCP make choices that are good for you.
As for the SSI, or SSSI, check and see if lawyers in your state receive set compensation, and no futher hold on you income, then hire one. You need to stop work ASAP, and a lawyer is the fastest way to get approved. Otherwise, I would say to apply once yourself and see.
I do not know if any Rheumy from father away would be willing to assist your PCP, but it would be great if they could look into that.
