By far the most common form of pulmonary embolism is a thromboembolism, which occurs when a blood clot, generally a venous thrombus, becomes dislodged from its site of formation and embolizes to the arterial blood supply of one of the lungs. Symptoms may include difficulty breathing, pain during breathing, and more rarely circulatory instability and death.

I've heard this theory before that the blood flow is so slow that athletes can actually be more susceptible to clots when doing things like sitting still or sleeping during long plane flights. (Serena Williams developed her PEs after a plane flight, although she also had a foot injury.)
I think really, that the conditioning isn't necessarily the cause but it could be a factor, for sure. It'd be better for everyone if they were aware of the need to get up and move around during plane flights and car trips ... regardless of conditioning.

I think it's a combo of low heart rate (my resting rate is about 45), dehydration during events and travel to /from events.
