By far the most common form of pulmonary embolism is a thromboembolism, which occurs when a blood clot, generally a venous thrombus, becomes dislodged from its site of formation and embolizes to the arterial blood supply of one of the lungs. Symptoms may include difficulty breathing, pain during breathing, and more rarely circulatory instability and death.
Were you ever tested for blood clotting gene mutations, disorders or other illnesses connected with heightened risk for blood clots? Even though Yaz may have been the leading factor in your PE, I wouldn't be surprised if there was something else that contributed, especially considering you also have a history of miscarriage. If you weren't tested, or perhaps not tested for all issues, I definitely suggest having tests run first.
