I am currently disabled due to PTC and another condition, so things might be slightly differnet.
I'm not sure if you work or nowt, but if you do you may consider applying ofr an FMLA leave of absence. I did this and when my FMLA time ran out, my job converted my leave to a long term leave of absence. After 6 months, my job has requird that I file for SSI.
I am now waithing on a decision from SSI.
I think this process is similar for most employers.
If you are not workiing you can just apply for SSI oever the phone, online or at a lcoal office. There is a 5 month waiting period before you will be paid (retroactievely) any benefits and a 90 day waiting period for a decision.
If you are going to go directly to SSI, I suggest checking oust the ssa.gov site. It will tell you everything you need to have with you to apply.
FMLA also protects your job for 12 weeks (I think).
