Abuse is a general term for the treatment of someone that causes some kind of harm (to the abused person, to the abusers themselves, or to someone else) that is unlawful or wrongful. No one deserves abuse, period. Abuse can be emotional, physical, or sexual.
As for your insurance, I have a bunch of questions (sorry).
Who said they would resubmit? The ins co or your employer?
Does your employer pay part & you pay part of your health ins premium? Even if you pay it all, do they deduct it from your paycheck?
If they pay any part of your ins premium and/or they normally deduct any part of it from your paycheck and they have been doing it consistently, then I believe they would have to be responsible. Those are just my thoughts...I'm not an attorney, but I would sure ask one.
I would also contact my state's labor & insurance commissioners to see what they say.
I would do all that before I approached approach the insurance company to appeal the claim citing that they covered the scripts in the same month.
I am holding the intention for you that all goes well at your dr's appt and that your employer and ins co resolves the ins issues easily and quickly in your favor.

Can you call the insurance company directly about these issues? You should have someone help you with all this, as it is the last thing you need. It is remarkable that you are keeping up your end of life, going to work, taking care of your home and your dogs. I, too, think that if the scrips were covered, the therapist should have been as well. It seems like a computer glitch, and in any case, was not your fault.
Good luck with your new gynecologist. I will say one thing about menopause: If you are going through a break-up with an abusive partner, it's like: What was that flying thing? (Superman?) I'm sure I shouldn't speak for all women, but I barely noticed. Surviving abuse, I think, must be so much worse.

Her secretary has been resubmitting and getting claim is not covered. Not sure how it's worded without paperwork in front of me.

My little Jack Russell was sick Friday after I got back from Chiro, migraine shot and then on to work. I tried to get her in after work but vet was not in. I ended up making an appmt Saturday morning and they fixed her up with antibiotic shot as she had a temperature.
I have been giving her antibiotics, feeding her boiled hamburger, rice and a bit of cottage cheese on top as well.
She seems to be doing better.

As for the gyno, will be awaiting results. They have me scheduled for a ultrasound on the 27th along with a mammogram. It has been at least 4 years since I have had a wellness check. I had dropped a kennel on my foot once moving it, then again, moving another time and both times hit my big toes. Both are still black and blue and this was weeks ago. The Dr did some blood work to check for diabetes because of that and when she heard I bruise easily and they take forever to go away.
She upped my celexa to 40mg and said Anxiety and Depression go hand in hand. I really did not want to go stronger on depressants as the whole weight gain thing.
I guess we will see.
Most large companies take your portion of the premium right out of your paycheck. That's what mine does.
My daughter's therapist took a vacation at a time when my daughter really needed the support. It sucks that they need vacations too!

It just stresses me as the last 3 visits I made, the receptionist approached me about the non payment and yet when I offered to just go back to paying what I paid before insurance, they said no, just pay the co-pay and we will keep trying. I also offered to cancel my appmts or not make anymore until it cleared up.
As of now, she will not be seeing me till Nov 4. So my anxiety is a bit high as I am maybe dealing with some dog business this weekend after work Saturday, then I will have sunday basically to catch up around my house and kennels, then back to work and off long enough for mammogram and ultrasound. Meanwhile I am waiting on bloodwork as the Dr felt I could be diabetic.
In the meantime, hoping Insurance is going to cover all of that...I know the premium was paid, but not very trusting now...
