The max dose of sinemet per day is 2000 mg of levodopa and 200 mg carbidopa....that is 1 25/250 8 times a day. You are at 400 mg levodpa a day and 100 mg carbidopa...you have a long way to go to get to 1400+mg levodopa. Your sitting pretty well from my point of view.

But at this dose you will likely be having dyskinesias like crazy. And that will be what limits your ability to increase at a certain point. You may not actually tolerate 8, 25/250 a day.........(When 25/100 stops working you will go up to 25/250.)

Therapy should be individualized and adjusted according to the desired therapeutic response. At least
70 to 100 mg of carbidopa per day should be provided. When a greater proportion of carbidopa is
required, one tablet of SINEMET 25-100 may be substituted for each tablet of SINEMET 10-100. When
more levodopa is required, SINEMET 25-250 should be substituted for SINEMET 25-100 or SINEMET
10-100. If necessary, the dosage of carbidopa levodopa 25-250 may be increased by one-half or one
tablet every day or every other day to a maximum of eight tablets a day. Experience with total daily
dosages of carbidopa greater than 200 mg is limited.

sorry...1400mg levodopa a day is starting to get high......from what ive read which is quite a bit, 1400-2000 mg levodopa per day typically is dyskinesia territory if not sooner.

Thank you for your information. Seems that I am taking a small dose. Was a little worried about taking the extra 2 1/2's at night. Do not want to get dyskinesia. Will try to remain on the dose that I am taking for a while. Just gotta start exercising more.

try avoiding a dose increase by exercise increase is worth a try. !!! stay low as long as you can......good luck.! sounds like your hanging tough.

increasing a dose often gives a burst of energy but it also makes some more somnolent. .....more tired. It can go either way. For more energy drink green tea....its a super antioxidant and gives you a burst of energy. Sencha is good so is Korean Ginseng tea.....all of these dont have the crash effect of coffee...or not nearly as much.

Birdy, You might suggest to you neurodocs to change your before bed dose from regular c/l to extended release so that you wake in am with some left in your system. That was my regimen for about 2 years

My husband's MDS doesn't increase the dosage which is 25/100 x 4
because if you increase it now, then in the future when it is needed more, then it won't work.
He has been on this dosage for 8 years now.

Lynne, Nobody knows the future. but we do have some control of the present. If you are unsatisfied with a doctor telling you to save more sinamet for future needs. Tell him the future is now. He is not getting adequate control and he needs to increase sinamet or add something else to the mix. He sounds like the old school of thought regarding sinamet.

I know what you are saying about increasing the pills, but I wish they could give my husband something else. Right now he is having hallucinations and twice this week he didn't know I was his wife.
I don't know if it's the Sinemet or dementia setting in.
His family doctor said if the hallucinations get worse they can give him something.
We don't see the MDS until Feb. so we'll talk about things. I just wonder if they can take him off Sinemet and put him on something different. We tried by our doctor suggestion to decrease by a half pill but his mobility was worse. We only did it for 3 nights, so I am afraid to do anything else until we talk to the MDS doctor.
It is just scary for us both when he doesn't know who I am.
We both cried this morning..........
