I was told to take it with food, but now i take it on an emmpy stomach, i feel good, and it helps greatly, this took about 3 months to feel that way
I have been taking Sinemet just shy of two years. Fatigue and being tired had slowly become a part of my life prior to taking Sinemet and has increased with time. I know many PD drugs can cause those affects, especially the agonists. I personally believe that it is a combination of both the PD and the meds we take for our PD, especially for those who have had PD for a while, even if they have only recently been diagnosed. Additionally, many people PD do not sleep well throughout the night, so that could also contribute to issues during the day. So, I just do what I can when I am at my best and sleep when my body tells me to, regardless if it night or day.
Good luck to you.
