I take mestinon every three hours.

Mestinon never helped this. It has improved with 3,4 Dap, plasmapheresis and rituxan.
I am fine during the day but by evening I have over-all fatigue, and am pretty much a couch potato. While sitting on the couch I use a foam travel neck pillow for support. As well, for sleeping, I use a neck roll pillow rather than a standard one.
