I guess the "Date Pain Nerve pain Joint pain" with "oct 4 6 5 2" underneath it is the dates of the month that one would suffer with the pain? I guess I just didn't understand the numbering, why 2 would come after 4 6and 5. It seems to me that if we are discussing calendar dates, they would be sequential.
The "Numbness/tingling Face hands legs" numbers below it represent the pain level 1-10 that they bother you? Sorry, I did understand what the chart is suppose to show to the doctor. I'm for anything to make it easier for the doc to remain interested in my MS.

Having a bad day so please bear with me.. this is what I understand it to mean.
The top row consist of the headings: 'date' 'overall pain' then individual pain levels for 'nerve pain' and 'joint pain'. Under the appropriate heading put today's date then give your overall pain level a number from 1-10 and so forth for the individual ones.
Her next set has headings of 'numbness/tingling' 'face' 'hands' & 'legs' and then under each one she has again rated the severity of each one on a scale of 1-10.
I think that is how she has done this. Of course each person would need to set it up to suit their needs.
Once I am feeling better I will try to create a chart in word or excel that I can email out if anyone is interested. Once upon a time I was the queen of flow sheets, charts etc.... not sure how good I'm at it now since the brain fog set in, but I'm willing to try. :)

So good point, Kayce, about your symptoms. I bring a list of questions and my neuro is so patient that he always answers all of my questions. I should have figured out the ortho doc because he didn't answer all my questions and rushed me out. That is a dead give away. Listen to your gut.
