I just say it's my adult ADD whenever I have a 'duh' moment in public. You are not alone!
I don't know what your problem is as I am not a doctor.
I suggest you seek a few opinions to find out what is wrong with you and no, I do not think it is "all in your head".

About doctors, once you're 'sure' you're not imagining it, though, I wouldn't let anyone say 'it's all in your head.' Any neuro who tells you that should be fired on the spot.
Doctors go through and do the tests and make a dx according to their knowledge of an illness.
If they don't notice anything unusual, instead of saying they don't know why you're having your symptoms, some will just say 'there's nothing wrong' and that anything you feel is something you're imagining.
Trust what your body is telling you. But I hope you can get some answers, too!

PS I am hard of hearing usually , but now as I have mentioned in other threads- AT times I have super hearing and noises are very loud-Lawn being mowed down street, car outside on my Avenue hitting pothole and sometimes this loudness really makes me fell weird.Stacey on this site has also mentioned loud noise bug her.Sorry I went off original topic-I notice I do that alot.Sorry-Ellen
