Migraine is a neurological disease, of which the most common symptom is an intense and disabling episodic headache. Migraine headaches are usually characterized by severe pain on one or both sides of the head and are often accompanied by photophobia (hypersensitivity to light), phonophobia (hypersensitivity to sound) and nausea.
I think that you need to make up your own mind about whether you need a carer and who that person ought to be. It may not even be relevant in your own mind right now. You may have to accept that your friend wants to move on but it is not your fault if she does. If she looks like staying, I guess you will need to decide if her disregard for your migraines is acceptable to you. I wish there was an easy to deal with this, but I think there is not. I do expect that good friends would listen even if they do not entirely understand everything that is said. Hmm.
