My suggestion is to first bring this to the attention of your doc. You do have some underlying medical issues that could potentially affect sleep. Your doc is the best one to advise you about this.
That said, the majority of insomnia is primary, meaning without a true medical basis. The true root of primary insomnia typically is in some combination of bad sleep habits and excessive worry about sleep, among other things.
Both of these issues can be sucessfully addressed through cognitive behavioral therapy for insomnia. This might be worth checking out if you haven't already.
