If I were signing that document and my H weren't around, I would probably put my parents down, thinking that he was obviously the primary guardian so this would give them an extra person to contact in case they couldn't reach him, or he was too involved with funeral arrangements, died in a freak accident, etc.
If you get an answer that you're not happy with, THEN call the MC. Consider this a pop quiz to see how much you've learned so far! Nothing is going to come of that form now, so it's a dead issue. But talking and finding out her reasoning is relevant and a good exercise in communication.

In most cases, the law will not just give guardianship to anyone, so quickly, unless the parents are both deceased, or incapable of raising that child. Unless you have some court records which state heavy addiction (which can still be appealed if you are in recovery), or physical abuse (documented), and possibly some time of emotional abuse (documented), along with some kind of DSM dianosis, like psychotic! So, don't fret too much about that issue.
But yes, she shouldn'thave madae this decision unilaterally, unless she wants to brew a storm. Yup, in counseling would be the best probable outcome, as opposed to your discussing it with her. Now if you guys are in the process of divorce, and she plans to move up there or something, that's a whole nother issue, also to be prepared to discuss with the therapist.
But Lisalisa made a good point! This sure does upen up the opportunity for you to, refine your communication skills, and to try to encourage hers too, cuz this is no way to find something like this out! Sorry, that it came to this!

And to complicate things worse, our son has dual citizenship like she does. She had him up there so he would get the dual.
And even worse being we are not Legally married, i do not know that even though i am on the birth cert as the father, that would trump her written statement.
