I'm going for it! I maxed out my leave for the year and my company wants me to fill out a short term disability form and a fmla form.
Fmla is not problem, my current doc is a surgeon, not a baby doctor so it is not too intrusive, but the short term disability form? That is another story! Verrrry personal.
I was so scared originally. Have you done this before? How did it go?
My employer asked me to return the form to them, but I know better. It has very personal questions! Information that my employer does not need to know darn it. So I called the insurance group and they agreed that the employer does not need to have a copy of the forms.
So I am going to go for it! 3 surgeries this year have added up for sure. I have taken a lot of time off from work. Anyone else in this boat?

I get hesitant about sharing much info with my employer, so I totally agree that it is none of their business exactly why...just that you have some medical stuff that requires time off. My immediate boss is aware only because sometimes, I have to take off at the last minute and I feel like I should explain (though I probably don't have to). Fortunately, she is a pretty good boss and tries to be supportive (even though she totally doesn't get it).
