HI there. I went through a similar experience. After my husband gave a sample we got the call that we "would never conceive naturally." It was a huge shock to us. My husband has a count that falls in normal range MOST of the time but for us, motility is the big issue too. We tried several rounds of IUI but were told chances were slim. We also made an appointment with a well known urologist but he really didn't have any answers for us. He said that there was a possibility that my husband had a varicocele which was causing the low motility. There is a surgery to repair that but the results are 50/50. Some get better and some don't and you have to wait at least 3 months to see if it helped. My husband's motility improved slightly by taking antioxidants, vitamins, not taking baths or putting his laptop on his lap, etc. We still ended up need IVF. So I would say, IVF with ICSI is your best bet. There are many women who are successful on their first round of IVF & ICSI when they are only dealing with male factor. For us, unfortunately, that wasn't the case. We are still trying.
I know its a big shock at first, but at least he has some swimmers in there and you have something to work with. Good Luck.

What was his motility at if you don't mind sharing? Did he get the surgery for the varicocele vein? How many rounds have you been through? It's so frustrating that we have to go through all of this! I can't stop saying 'why us'. Just last week, one of my husband's student told my him he was going to make a great father! If only I could just snap my fingers and make it happen instead of having to go through all of this crap. It's all so surreal...

So, our next step is meeting with the urologist, so we'll see what he has to say. I'm pretty my hubby will have to give more samples to be sure that the first readings were accurate. I'm also going to be meeting with my gyno. I'm sure there's a wack of tests that i have to do too before we go on with IVF.

My DH has severe male IF, I also have multiple IF factors. When we first found out all of our problems my husband was referred to a Urologist. We knew from the beggining we would need IVF with ICSI. My DH's sperm analysis was almost nonexistant when we first started all of this. He had extremly low volume and count and no motility. His tesosterone level was also low. The Urologist started him on Clomid, he has been on it for almost a year now. Wow!! the changes it has made in his sperm analysis.
Everything improved and although its still not considered "normal" its a vast improvement. His volume is now anywhere from 0.3-0.5ml, total sperm count is 8.4 million, and he now has progressive motility.
His testosterone is now high normal. We know we will still have to do IVF with ICSI and are currently in our first cycle!!
All of the doctors have been impressed and pleased with the improvement and of course we are thrilled also. Good luck, hope this helps.
