I bought 4 vials, and I paid 2500$ for them... still waiting to get my first insemination..
I'd call Columbia's woman center in NYC and Dr. Mark Sauer.
I went to a seminar and Dr. Sauer said there's no such thing as male infertility anymore. They'll go right into the testes and pull the swimmers out.
Again, I apologize as I don't know the whole story...but I really don't trust RE's all that much and I would definately get an opinion.
If you do go with sperm donation, Fairfax, California Cryobank and Xytex are the largest. I'm trying to find one in NYC so I can carry my own sperm to the office as the shipping costs are like $150.
Vials run for $400-$600 depending on open identity (which is what I'm doing) and education level.
There's also Pacific Reproductive - my fav - but no RE works with them that I know of. They're lesbian-owned and will ship to the home for you to inseminate yourself.
I think yourself and your DH need to check out Columbia and then really decide what you want to do.
