With the MRI issue, I don't really know what to think about it and I am unable to find out much information on it. What I do find is not cute.
The "complex" migraine.... Well, they upped my Imitrex to 100mgs and I'm still on the Elavil 30mgs. They are apparently a pre-cursor for strokes, in some doctor's eyes.
You may want to get more labs done as these are now too old. I do not see any thyroid labs of the Free T3 and the Free T4 being done. However seeing how high the TSH was back then for you makes me wonder if your Free Ts are too low for the problems you are having. Low Free Ts can contribute to Insomnia issues.

I'm glad you've had further testing and sorry to hear you're still struggling.
I'm glad they're looking at your adrenals since problems with those could have caused your issues with thyroid hormone replacement.
Without a doubt, your hypothyroidism could be causing or contributing to your migraines so, addressing your hypoT is a good idea as mm recommended.
