Hyperparathyroidism is overactivity of the parathyroid glands resulting in excess production of parathyroid hormone (PTH). Increased PTH consequently leads to increased serum calcium (hypercalcemia) due to 1) increased bone resorption, allowing flow of calcium from bone to blood, 2) reduces renal clearance of calcium, and 3) increases intestinal calcium absorption.

Well, I had my pre-op calcium/PTH test. My calcium was 9.4 and my PTH was 38. My Vitamin D is up to 25 from 18. How can this be? I need to go in tomorrow and have my ionized/PTH/serum and D pulled again, but barring a lab error on this last one, how can I go from elevated ionized, elevated PTH and hypercalciuria for over a year, to these values in less than 2 months? I know the surgeon will likely want to cancel the surgery and I am depressed beyond belief. All this hope for nothing.

Thanks for asking. The retested number showed a 9.8 serum and a 5. something ionized (still in normal range) but an 80 PTH. My D was up at 25 from 15 before. She is still going to proceed based upon the hypercalciuria and PTH. If this doesn't solve it, then we still have the hypercalciuria to figure out so that I don't get stones and eventual kidney failure.

Surgery is tomorrow morning early. Hopefully this will accomplish something. If not, back to the drawing board....
