Have you tried any of the elimination diets or the Autoimmune Protocol Diet (AIP) to try to identify a potential food trigger? I put it off for years. I was so sure the doctors and medicine had the answer for HS. When I couldn't take it anymore I did a modified version of AIP and figured out my trigger foods are ones that contain wheat gluten. No more wheat based foods for me, but at the same time HS flares are now a rarity (unless I get glutened by accident). I've eaten a ton of wheat based products my whole life without any problem until a number of years ago when the HS started. It took another 3 years before I realized modern medicine did not have the answer. If you haven't tried AIP (or a modified version), its worth a try. The worst that can happen is things stay the same. The most commonly reported food triggers are nightshades (potatoes, tomatoes, peppers, paprika, tobacco), wheat gluten (breads, pasta, pizza crust, anything with wheat barley or rye products in them), dairy (milk, cheese, butter, yogurt, etc), and nuts.
