Open up to those around you about the issues. If it's that debilitating that you can't stand for more than 20 minutes you probably need surgery - assuming you don't live in a 3rd world country it should be government funded? Maybe start heading down that path and ask for support to get you through the surgery so you actually have a goal to improve the condition.
Also everything seems to point to diet.... I have never been a dieter, but I can demonstrate flare-ups 12 hours after eating potato.
