I did this once..I was at a bf's house for fathers day..his dad was coming in and had some things to carry..I told my son, "Go help Johns dad carry in his things" too bad I was at DALE'S house!!!
I too felt horrific! But, I simply wasn't over the previous relationship. I had not given myself time to heal my heart.
We all want to be able to move on and find someone special to be with, unfortunately when we don't allow ourselves the space and time to heal, this is one of the things that happen.
I'm sorry this is happening. Just slow things down.....and don't be too upset with him..I'm sure he was even more mortified and embarrassed then you may think...

I didn't let it hurt me too much, I simply took it as, for the past 10 years, that is the name he has always said, and just slipped back into routine.
2) That's why I call people "Hon" or "Darlin'" or Sweetie or something like that! My daughters are Michelle and Melissa and I'm constantly calling them each other's names.
He probably didn't mean anything by it. He said her name for the past 10+ years, so it was something out of habit- not meant to insult you or to mean that he is thinking of her while with you.
Give him a break and hopefully he learned from this mistake and won't be making it in the future.
I would take ti slow regardless and work with it. It doesnt mean much sometimes when this happens.
