I had an extended tummy tuck and breast lift with augmentation 9 months after my RNY (and before I hit my goal weight). Almost a year later, I had my backside done (which also refined the original tummy tuck). A few months after that, I had my arms done Five months later, I had my legs done.
No, insurance didn't cover any of it. Yes, I'm still making payments - but that's okay with me, because I get to live in the new body while I do!
If you choose to have plastics, I highly recommend going to the plastic surgeons in your area who perform surgeries on the "women who do lunch" crowd. Here in AZ, that is the Scottsdale clientele. I believe surgeons accustomed meeting the high demands of very picky women, perform more elegant work than the plastic surgeons who "specialize" in skin removal for extreme weight loss (when almost anything they do is an improvement.
If you can only afford one surgery, I recommend doing the tummy tuck and breast lift. The TT does more than remove the excess skin. It repairs the stomach muscles. Pregnancy and obesity cause the linea alba, the tissue that connects the right and left abdominal muscles, to separate. No amount of exercise can repair the damage. The only way to fix it is through surgery.
