I looked into the surgery, too, but decided against it. Too many experts recommending no elective surgery as it could throw me into a big time flare.
I think weight can be a problem for many of us because we don't have the energy to exercise and for me, it's fear of hurting myself by doing exercise. I will restart a physical therapy program--aquatherapy, again soon.
I feel for you being in so much pain. I'm sorry you are feeling so bad. I too have really bad sciatica. I recently was diagnosed with si joint dysfunction. I am and was in so much pain that I cannot sit in a regular chair for more than 10 minutes without having to get and if I can find a chair to sit in for a long period or am on pain medication to do so, then I can hardly get up from the seated position. You may want to see a Rheumatologist to see if you have SI joint dysfunction or another condition. I also have bursitis in my hips I found out which is also causing so much pain I cannot sleep.
I have had epidural steroid injections in my low back. I actually had 3 before the relief came. The first one was not in the right spot and then they had to do 2 bilateral injections scheduled 2 weeks apart before I got any pain relief. I think weight is an issue, but it's not the cause of the pain. Sure, I might have "some" relief from losing weight, but I still have the pain and the fibro makes it even worse than for a normal person. Some docs just don't get that. On an image things might look normal, but my highly recognized and awarded Rheumatologist even told me that Fibromyalgia is more painful than even Ankylosing Spondylitis. I finally found a doc that "gets it". I hope you find one too or your pcp relaizes how difficult it is to deal with this painful condition.
So, In essence, don't be hard on yourself. Be kind to yourself and your body. Give it what it needs, the comfort and care you would give to a sick friend. Don't let outside influences "tell" you how should feel or what you should do. Listen to your body and try to be gentle with yourself in losing weight or trying to exercise if you can. You know your body and your limits.
