I hope it's ok to post this question here, I just don't know how it goes. I am almost done filing for SSDI, and I have spoken with my Dr;s offices and they say,"make an appointment and bring in the papers for us to fill out." I am doing it all online, but may take the signed paper in, instead of mailing it. So when I do that will they give me the papers for the Dr's to fill out, or is there a place online I can print them????

I hope I just made sense, having a very "fog" filled day.

You need to ask SS if there is a form to be filled out. Sometimes they will contact the doctor directly.

I made sure the doctor signed a letter of disability for me and put it in my file so when SS asked for the records it would be part of my file. I actually typed the letter myself and asked him to sign it so it would save time.

I never gave my dr anything to fill out. He was sent papers. I think the bunnys way is a good way to go about it.

Do you have a lawyer? That's who should be getting information from your doctor. My lawyer gave me papers to take to them. He sent them twice but the doctor never replied. I had to hand carry them.

BTW, I lost my case and eventually went back to work, but... I work in the family business so it's not like working in the real world.
