was reading diagnosis list in ny chart recently noticed thyroid dis function listed. have thyriod check regularly never a problem. asked my pcp about in we discovered that someone had put wrong recorded in wrong we both have same name silmaler history but the dob are differant. so i advise you to read all charting carefully.
I just remember getting a frantic call that I was getting an "urgent" referral to a hospital for further testing (some kind of procedure they were sending me for) and I went into full panic mode. I physically drove to the clinic and asked them to SHOW me where it says I was diagnosed with this disease and she printed it out but it didn't make sense to me. Even the doctor at the hospital I was getting referred to couldn't understand why I was sent to her. But she still did the procedure anyway just to make sure I was healthy.
Anyway, it wasn't till months later after continual digging that they finally admitted to me "Oops.... that wasn't supposed to be in your record. Sorry for the inconvenience you had to go through. My bad." I was furious! I wanted to get a lawyer and sue the pants off them but I can't since it's govt healthcare (the VA clinic). Those a-holes could have accidentally had my uterus removed and been like, "Oops... my bad, that was for another patient who had cancer, not you. Sorry for the inconvenience." And the govt would have just gone on their merry little way. Makes you wonder about the patient whose record it was SUPPOSED to go to. Did she get her cancer screening??? OMG
