Finding a job is much easier when you have one. Get out now.
Also, consider going part-time there and part-time elsewhere (yes, even the dreaded McD's if necessary) and just focus on the fact that in a few months---it's all over.

What that MIGHT do is postpone the inevitable, because if you identify yourself as disabled and ask for accommodation, anything they do immediately after is going to look like retaliation, which is blatantly illegal.
It won't protect you from misconduct (cell phone) or actual performance issues, but if you're careful with all that, it might buy you some time to get yourself to a place where you leave on your own terms.
