Family issues is a huge range that go from minor conflict to major misbehaviour and even abuse on the part of individual members of the family occur. I think at that age, the conversation should be rather quick. Basically when your children bring it up, and they say that mom said she was sick, you should just say that it's a shame, and change the subject. That way you're not acknowledging, not sticking the kids in the middle of it, and not giving attention to a situation they really aren't old enough to understand.
I think I would reply differently if your children were older, however they're just not able to understand, and if they do, they're young enough to say something to her, which will only turn them into the rope in a tug of war competition.

First of all if you think she could be impaired while around your children I would make very sure she is sober when she comes to pick them up or leave them. Tell the kids you have to "talk" to their mother about something and check her out before she takes them.
Then tell your daughter that if she feels that mommy is acting "sick" when she is visiting that she should call You or Grandma or someone so that they can help Mommy.
My daughter had a friend in this situation. The principal noticed that the Mother was acting high on something and refused to release the daughter to her one afternoon.
In any case, kids are smart... I'm sure your daughter already knows a lot more than you think she does.
I would also suggest getting an order from the court that she has to have supervised visitation and can absolutely not drive them anywhere. What would have happened if she had taken those pills and then made it to the school to pick them up??
