I think the judge in the charging state would be the one who can change the conditions but it's such a mess. Do you have an attorney?

The sentencing state is going to give you conditions of probation which you have to follow. This is where it gets confusing - the receiving state doesn't have to follow those conditions and can impose their own BUT you still have to follow the sentencing state's conditions in addition to anything the receiving state adds otherwise you can be violated.
In practice if the receiving state isn't monitoring a condition imposed by the sentencing state the odds of getting violated for that condition are very slim but do be aware it could still happen. As an example Florida usually imposes driving restrictions like you can't drive alone and have to maintain a driving log. If you transfer out of Florida most receiving states won't impose those conditions so you could drive alone and not keep a driving log but Florida could still violate you for not complying with those conditions.
When I took my son out of Floriduh I emailed the police station to make sure he wouldn't have to register there before coming in Philly and I got an email back so I have it in writing. Get everything you need in writing if you can.
@FreeToBreathe - Is the sending state refusing to terminate your probation for some reason or do you have to petition the court to terminate it? If had completed the full sentence over a year ago I would definitely be filing something with the sentencing court by now seeking a court order acknowledging to termination of probation
