I would also suggest that with all the conditions you have, water exercising would be excellent for you. The PT in water is supposed to work wonders for so many people and since you already have had some instructions, it would not be hard for you to replicate those movements on your own. I hurt my knee in the past and while PT helped a great deal, it sure was expensive. I know which movements helped me, so I don't need to pay to have someone watch me do the exercises.
The thyroid issue is also a concern when it comes to weight issues; you could look into any need for some adjustments or changes to that medication. It may make a difference to your exercising approaches.
When seeing a personal trainer at the gym, be sure to take some reports on your back conditions and other issues so that you can be matched with a trainer who is knowledgeable enough to be able to work with your conditions. Because of your limitations, it would be more productive to inform the trainer so that your routines can be tuned to your specific needs.
Last, for your eating guidelines, you may want to consider seeing a nutritionist or dietitian for some tips or a plan to get you started in a way to support a permanent and healthy maintainable weight loss.
